underserved and underinsured children for enrollment
into the State Children's Health Insurance Program.

Community Health Access Network

This grant enabled the development of
a coordinated chronic disease management and education
system among seven community health centers for diabetes
and asthma, with an emphasis on provider and patient
education. This was the second year of support for this
project.
Greater Derry Community Health Services
Location: Derry
Grant Award: $15,000
Grantmaking Area: Barriers to Enrollment and Retention
Area Served: 10 communities in the Derry area
This grant provided operating support
for a service network of for-profit and nonprofit organizations,
helping to expand and improve access to comprehensive
primary health care and specialty referral services
for individuals that do not qualify for public assistance
programs, but for whom the costs of commercial health
coverage are out of reach. This was the second year
of support for this project.
The North Country Health Consortium
Location: Littleton
Grant Award: $9,000
Grantmaking Area: Evaluation
Area Served: Northern Grafton and Coos Counties
This grant funded the evaluation of the
feasibility for a system that would coordinate health
services to reduce the costs of health care for North
Country residents. This was the second year of support
for this project.
