a coordinated chronic disease management and education
system among seven community health centers for diabetes
and asthma, with an emphasis on provider and patient
education. This was the third year of grant support

This grant funded the 35% non-federal
share of the State Children's Health Insurance Program
for the first six months of CY2001. The second six months
of the grant funded the 35% non-federal share for children
whose family income was between 250% and 300% of the
federal poverty level. This was the third year of grant
support for this program.

This grant funded the evaluation of the
feasibility for a system that would coordinate health
services to reduce the costs of health care for North
