This grant provided matching funds for the Robert Wood Johnson Covering Kids and Families Initiative. The grant provided outreach to children living in minority families in need of services to encourage enrollment and retention in the State Children's Health Insurance Program.

This grant enabled the development of a coordinated chronic disease management and education system among seven community health centers for diabetes and asthma, with an emphasis on provider and patient education. This was the fourth year of grant support for this project.

New Hampshire Department of Health and Human Services

This grant funded a portion of the 35% non-federal share of the State Children's Health Insurance Program for children whose family income was between 250% and 300% of the federal poverty level. This was the fourth year of grant support for this program.

This grant enabled the comprehensive evaluation of the State Children's Health Insurance Program by analyzing data, developing trend information, linking enrollment to encounter data and beginning the work of setting quality improvement goals.
