Commuity Transformation Grants (CTG)
 
KEY PROVISION: Local communities will address health disparities and reduce chronic disease by promoting healthy living and addressing the root causes of poor health.     

Through an authorization in the Affordable Care Act, the Centers for Disease Control and Prevention (CDC) will award funds for the implementation, evaluation, and dissemination of evidence-based community-preventive health activities in order to reduce chronic disease rates, prevent the development of secondary conditions, address health disparities, and develop a stronger evidence base of effective prevention programming.

Why this is important
We reduce health costs for all residents when we eliminate health disparities that are the result of race, ethnicity, social status, economics and geography.  CTG will be used to create healthier school environments, communities and worksites by creating safe places to walk and be active and increasing access to healthy food for all our residents.     

What this means in New Hampshire
In July 2011, an application for funding to support Community Transformation Grant implementation activities in New Hampshire was submitted to the Centers for Disease Control and Prevention. Grant award notification is expected in late September 2011.
